Work on flexibility in your thoracic spine.
Multiple problems but the easiest one to fix is probably your stance being open. Compare your two toes relative to your target line. They should be parallel or you could even try a closed stance if you want to encourage in to out to hit a draw.
Second easiest fix is to strengthen your grip.
From there you’ll probably still be slicing but likely not nearly as badly.
Very similar to me awhile back - You’re actually aimed left of target with an open stance and likely an open clubface = very hard not to have an out to in (over the top swing).
Start by dropping the trail (right) shoulder back and a little lower swing more out to the right.
Early extension with lack of full rotation in backswing seems to be the issue.
Feel like back is facing target at the top of the backswing to fix the rotation issue. For early extension focus on wall drill, and check out Erik Cogorno. These alone should help to sort the swing path and get you hitting straighter shots.
Turn, turn and turn. Try to turn the hips and the shoulders more; that might already fix it.
Your club is going out to in. Square the face and throw the club head at the ball. The iron swing and driver swing are not the same.
As you transition through your downstroke and you start to rotate your body, your left elbow should then try and hug the left side of your waist and the club will take a shallower line up to impact. It looks like you are trying to push the ball to the target instead of letting the swing and club face make impact with the ball.
This may cause some left hooks initially, but then you need to sort your lower rotation.
